(DE HOPE)

OUR PROGRAM

The Delaware Healthy Outcomes and Parent Engagement (DE HOPE) program is a multidisciplinary home visiting program designed for infants with prenatal substance exposure and their caregivers.  DE HOPE team members work collaboratively with Delaware's Medication Assisted Treatment (MAT) providers and the Division of Family Services (DFS) to promote child well-being, increase permanency outcomes, and enhance the safety of children at-risk of being placed in out-of-home care as a result of a parent or caregiver's substance use disorder.

TARGET POPULATION

Pregnant women and new mothers who seek or are enrolled in Medication Assisted Treatment (MAT) during their pregnancy or when their baby is less than three months of age, for up to three years,
